Notice of the making of this Statutory Instrument was published in

“Iris Oifigiúil” of 17th December, 2010.

I, BARRY ANDREWS, Minister of State at the Department of Health and Children, in exercise of the powers conferred on me by section 150 of the Adoption Act 2010 (No. 21 of 2010) and the Health and Children (Delegation of Ministerial Functions) Order 2010 ( S.I. No. 508 of 2010 ), hereby make the following regulations:

1. These regulations may be cited as the Adoption Act 2010 (Section 134) (Forms) Regulations 2010.

2. The collective title “the Adoption Regulations 2010” includes these regulations.

3. (1) In paragraph (2), “notice” means the notice of every registration, or cancellation of registration, of an accredited body in the register of accredited bodies which is required by section 134 of the Act to be published by the Authority in Iris Oifigiúil.

(2) The forms of notice are set out in the Schedule.

4. These regulations come into operation on 14th December 2010.

Form 1

NOTICE OF REGISTRATION OF ACCREDITED BODY

The Adoption Authority has registered..........

(name of accredited body) in the register of accredited bodies.

Form 2

NOTICE OF CANCELLATION OF REGISTRATION OF ACCREDITED BODY

The Adoption Authority has cancelled the registration in the register of accredited bodies of ..................................................................... (name of former accredited body).

These regulations set out the forms for the notification by the Adoption Authority of the registration of an accredited body or the cancellation of the registration of an accredited body in the register of accredited bodies for the purposes of publication of that information in Iris Oifigiúil.
