 This form is for use only by Tutors working under the terms of the Departm ent of Education Home Tuition Scheme.

It is not for use by Tutors working under the terms of July Provision/Summer Education programm es or by

Teachers/SNAs/Clerical staff/Caretakers in Schools.

 This form is only to be completed if the following circumstance(s) appl y:

o The Tutor has never previously been paid by the Department.

o The Tutor wishes to change the existing bank details held by the Departm ent

 The Department will operate one bank account only for each individu al.

 The details submitted on this form will supersede any already held by the Department of Education
