CORONERS ACT, 1962 (FORMS) REGULATIONS, 1962

I, CHARLES J. HAUGHEY, Minister for Justice, in exercise of the power conferred on me by subsection (1) of section 3 of the Coroners Act, 1962 (No. 9 of 1962), hereby make the following regulations :

1. These Regulations may be cited as the Coroners Act, 1962 (Forms) Regulations, 1962.

2.—(1) The oath to be taken by the jurors at an inquest shall be in the following form :

" I swear by Almighty God that I will well and diligently inquire as to the identity of the person in relation to whose death this inquest is being held and as to how, when and where the death occurred and a true verdict give according to the evidence."

(2) The oath to be taken by a witness at an inquest shall be in the following form :

" I swear by Almighty God that my evidence shall be the truth, the whole truth and nothing but the truth."

3.—(1) The summons to be served on jurors to attend an inquest shall be in the form set out in the First Schedule to these Regulations.

(2) The summons to be served on a witness to attend an inquest shall be in the form set out in the Second Schedule to these Regulations.

4. Every deposition taken at an inquest shall be in the form set out in the Third Schedule to these Regulations.

5. The record of verdict at an inquest shall be in the form set out in the Fourth Schedule to these Regulations.

6. Every certificate issued by a coroner under section 48 of the Coroners Act, 1962 (No. 9 of 1962), in relation to the removal out of the State of the body of a deceased person shall be in the form set out in the Fifth Schedule to these Regulations.
